I found the problem... I had added Text through "Edit Text & Images", when I switched to adding a Comment text, I was able to print the "Form Fields Only"

You CANNOT check resolution by opening in Photoshop, because when you open it rasterises to an arbitrary resolution not connected to the images. A SAVE AS will reclaim space from deleted pages but...
