What actually created this PostScript file or what you think is a PostScript file?
Is it possible that this is actually an intermediate PostScript file used in a Xerox VIPP workflow? If so, it is really not directly “Distiller-ready” so to speak.
The fact that it distilled with Distiller 5.0, released probably 13 years ago for use on Windows 2000 and Windows'98 means that when you ran this, if was in a very different environment that perhaps had a VIPP processing environment installed with additional procedure definitions in a Distiller prologue file.
